Tax Form

All National Sales Representatives must have a W-9 on file before beginning orientation. Please download the blank form below, complete it, and upload your signed copy.

  • Application Review:  We’ll confirm your information and receipt of your W-9.

  • Orientation Scheduling:  You’ll get an email to book your orientation, where you’ll gain access to all training resources.

  • Welcome Package:  After completing orientation, we’ll send your official LRW shirt and provide details on optional branded items.
